USA - Marine Casualty and Serious Marine Incident Reporting Forms

In March 2013 the United States Coast Guard (USCG) published new versions of form CG-2692 “Report of marine casualty” and CG-2692B “Report of required chemical drug and alcohol testing following a serious marine incident”, however, vessels continue to submit old versions of the forms.

Members are reminded that foreign flagged vessels within the territorial and inland waters of the United States, and vessels registered in the United States regardless of location, involved in any accident considered a marine casualty or serious marine incident are required to notify the USCG. Vessels trading to the United States and United States flagged vessels should have copies of the latest forms readily available onboard for this purpose.

If an incident falls under the definition of a marine casualty as listed in 46 CFR 4.05-1, then form CG-2692 “Report of marine casualty” must be completed by the master, owner, agent or operator and submitted to the USCG office nearest to the site of the casualty, or closest to the port of arrival when an incident has occurred at sea, within five days of the casualty. When a marine casualty also meets the requirements to be considered a  serious marine incident as detailed in 46 CFR 4.03, then form CG-2692B “Report of required chemical drug and alcohol testing following a serious marine incident” must also be completed and submitted to the USCG.
